29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about June 24, 2025, employees working on and around the Aeonic Press (Model No. HD-30s & Press No. 7) were exposed to pinch points and caught-in hazards, in-that the employer did not ensure the shuttle box/top plate was properly guarded. (b) On or about June 24, 2025, employees working on and around the Hydraulic Press (Press No. 6) were exposed to pinch points and caught-in hazards, in-that the employer did not ensure the shuttle box/top plate was properly guarded. (c) On or about June 24, 2025, employees working on and around the Hydraulic Press (Press No. 50) were exposed to pinch points and caught-in hazards, in-that the employer did not ensure the shuttle box/top plate was properly guarded. (d) On or about June 24, 2025, employees working on and around the Hydraulic Press (Press No. 12B) were exposed to pinch points and caught-in hazards, in-that the employer did not ensure the shuttle box/top plate was properly guarded.

(a) On or about June 24, 2025, employees working on and around the Aeonic Press (Model No. HD-30s & Press No. 7) were exposed to a snagging/caught on hazard from an unguarded rotating vertical shaft from the shaker motor, on the back side of the press. The employer did not ensure ensure the rotating vertical shaft was properly guarded. (b) On or about June 24, 2025, employees working on and around the Hydraulic Press (Press No. 6) were exposed to a snagging/caught on hazard from an unguarded rotating vertical shaft from the shaker motor, on the back side of the press. The employer did not ensure ensure the rotating vertical shaft was properly guarded.
