This exposed employees to struck-by and caught-in-between hazards.  (b)  On or about June24, 2025, the employer did not ensure that employees were trained and certified to operate the forklift (Flexi Model 1200, Serial No. NAO5488Z) at the facility. This exposed employees to struck-by and caught-in-between hazards.
