On or about June 24th, 2025, employees engaged in production activities located at 5401 Alvo Road, Lincoln, Nebraska 68514, were exposed to fall hazards in that employees working on top of brewery vessels and they were not attached to certified anchor points that were capable of supporting at least 5,000lbs.  Instances of failure include but are not limited to:   1.) Employees would secure their fall protection device to pipes attached to the brewery vessels.  2.) Employees were using a retractable fall protection device that was mounted to a steel beam on the ceiling in the production area.  The steel beam was not certified.
