29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level .  On or about June 24th, 2025, employees engaged in production activities located at 5401 Alvo Road, Lincoln, Nebraska 68514 were exposed to fall hazards greater than 7 feet in that employees were climbing on top of beer brewing vessels to mount motors to the vessels.

29 CFR  1910.140(c)(13)(i):Anchorages were not capable of supporting at least 5000 pounds (22.2 kN) for each employee attached.  On or about June 24th, 2025, employees engaged in production activities located at 5401 Alvo Road, Lincoln, Nebraska 68514, were exposed to fall hazards in that employees working on top of brewery vessels and they were not attached to certified anchor points that were capable of supporting at least 5,000lbs.  Instances of failure include but are not limited to:   1.) Employees would secure their fall protection device to pipes attached to the brewery vessels.  2.) Employees were using a retractable fall protection device that was mounted to a steel beam on the ceiling in the production area.  The steel beam was not certified.

29 CFR  1910.140(c)(18):29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service.  On or about June 24th, 2025, employees engaged in production activities located at 5401 Alvo Road, Lincoln, Nebraska 68514 were exposed to fall hazards in that employees were using yellow/black fall protection harness that contained frayed/damaged to one of the leg straps.  The employer was not requiring employees to inspect their fall protection devices prior to use.

On or about June 24th, 2025, employees engaged in production activities located at 5401 Alvo Road, Lincoln, Nebraska 68514, were exposed to fall hazards in that the employer failed to provide fall protection training to their employees prior to exposing them to heights greater than 7 feet.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about June 24th, 2025, employees engaged in electrical maintenance activities located at 5401 Alvo Road, Lincoln, Nebraska 68514 were exposed to electrical hazards in that employee were accessing the main electrical panels for the brewery vessels while the vessels were energized and not LOTO.  The employees were exposed to voltages that ranged from 3 phase 208-480v.  The employer failed to establish and implement a written LOTO program.

The employees were being exposed to 3-phase 208-480V.
