29 CFR  1910.23(c)(9): Portable ladders used on slippery surfaces are secured and stabilized;  On or about February 9, 2025, Gutters Unlimited LLC failed to ensure that a 28-foot portable ladder was secure and stabilized on the asphalt driveway located at Holy Trinity Orthodox Church, 8622 Broadview Road, Parma, OH 44134. The employer was installing snowbirds on the top of the car port when the ladder kicked out due to ice causing the employee to fall and get injured.

29 CFR  1910.30(b)(1): The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment.  On or about February 1, 2025, Gutters Unlimited LLC failed to train employees on adequate usage of ladders. An employee failed to ensure that a 28-foot portable ladder was secure and stabilized on the asphalt driveway located at Holy Trinity Orthodox Church, 8622 Broadview Road, Parma, OH 44134. The employer was installing snowbirds on the top of the car port when the ladder kicked out due to ice causing the employee to fall and get injured.

29 CFR  1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.   On or about February 1, 2025,  Gutters Unlimited LLC, did not report within 24 hours the in-patient hospitalization of an employee who fell approximately 15 -16 feet  off of a ladder at a worksite located at 6822 Broadview Rd, Parma, OH 44134.  Injuries sustained include multiple broken bones and strains.
