29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition:  (a) 2395 6th Street, LIMON, CO 80828: On or about June 25, 2025, and at times prior, employees working in the kitchen were exposed to trip hazards due to damaged, uneven, floor tiles.  This condition exposed employees to stuck-by hazards.

29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  (a) 2395 6th Street, LIMON, CO 80828: On or about June 25, 2025, and at times prior,  no written certification of a hazard assessment was available. Employees worked in a kitchen environment where hazards requiring the use of Personal Protective Equipment (PPE) were present, including but not limited to handling cleaning chemicals.
