29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) Jobsite: On or about July 7, 2025, employees using personal fall arrest systems during roofing activities of a church had their attachment point at the waist rather than at the center of the back on the harness. The employees were exposed to falls ranging in height from 12 to 20 feet.

29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: (a) Jobsite: On or about June 25, 2025, the employer exposed employees to electricity while permitting them to erect a shingle lifter close to a single phase 13.2 kV overhead electric line without ensuring it was effectively guarded. The shingle lifter contacted the uninsulated overhead electrical line resulting in a fatal injury.

29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take. (a) Jobsite: On or about June 25, 2025, the employer did not post warning signs or inform employees about the protective measures necessary to avoid contact with the overhead line before work began. Employees were standing up a 36 foot 4 inch long shingle lifter when its top contacted an uninsulated overhead single-phase 13.2 kV (phase to ground) electric line.
