29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) On or about June 24, 2025, at the above addressed jobsite, employees were exposed to fall hazards of  approximately 10 feet ground to eave while performing residential roofing work.  GXP Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1654214, citation 1, item 2, and was affirmed final order on April 14, 2023, with respect to a workplace located at 8712 N Olcott Ave, Niles, IL 60714.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)  On June 24, 2025, employees were using a portable extension ladder which did not extend 3 feet above the upper landing being accessed.  GXP Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1654214, citation 1, item 3, and was affirmed final order on April 14, 2023, with respect to a workplace located at 8712 N Olcott Ave, Niles, IL 60714.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
