29 CFR  1910.305(a)(2)(x): Flexible cords and cables were not be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.   a) On or about June 26, 2025, at the PFC Santos Aviles Reserve Center, Main Building, Lobby Area: an extension cord used to provide electrical power to an ice machine was ran under a door, causing parts of its outside jacket to expose the inner coated conductors, exposing employees to electrical hazards.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord set (extension cords) was not visually inspected before use for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).   a) On or about June 26, 2025, at the PFC Santos Aviles Reserve Center, Main Building, Lobby Area: an extension cord used to provide electrical power to an ice machine was frayed and had damaged outside jacket at various locations exposing the inner coated conductors, and  exposing employees to electrical hazards.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) On or about June 26, 2025, at the PFC Santos Aviles Reserve Center, Main Building, there was a fire extinguisher, Model Amerex 415, serial number AH650671, not mounted on a wall and missing a breakable seal, exposing employees to fire hazards.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.
