29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). (a) Residential Roof: On June 26, 2025, employees were performing roofing work on a residential roof with an approximate 4/12 pitch, at heights of up to approximately 19ft. 6in. above grade, without the use of a fall protection system, exposing employees to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: (a) Residential Roof: On June 26, 2025, employees used a Little Giant ladder to gain access to the porch roof. The ladder was not extended 3 feet above the edge of the roof, exposing the employees to a fall of up to 9ft. 11in. to the ground. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
