29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about June 18, 2025, an employee was exposed to a struck by hazard while performing roofing activities on a residential home located at 6906 Dora Blvd, Independence Ohio 44121.  The employee was using an air driven nail gun to install shingles on a residential roof and the employer did not ensure protective safety glasses were worn.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 18, 2025, an employee was exposed to a fall of approximately 11.1 feet while performing roofing activities on a residential home located at  6906 Dora Blvd, Independence Ohio 44121. The employee was working on the roof and the employer did not ensure fall protection was used.
