29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:   On or about June 26, 2025, employees were exposed to struck by hazards while installing shingles with pneumatic nail guns  on a residential roof located at 11613 Tinkers Creek RD, Valley View Ohio 44125, without wearing any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 26, 2025, employees were exposed to a fall hazard of 10.6 feet to the ground while completing shingle installation on a residential roof located at 11613 Tinkers Creek RD, Valley View Ohio 44125, and were not protected by guardrail systems, safety net system, or personal fall arrest system.
