On or about 6/25/2025, the employer did not protect employees who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels while at the job site located at 3766 Jeffery Ct in Arnold, MO. An employee was working from the edge wall of a scraps trailer conducting residential roofing operations that exposed them to the hazard of falling from a height of greater than six(6) feet.

29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the hole.   On or about 6/25/2025, at 3766 Jeffery Ct in Arnold, MO, employees were exposed to fall hazards while working around a hole in a roof without fall protection. Employees were removing bad sections of the roof wood panels and replacing it when exposed to falling approximately 10 feet through the hole.

The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury while at the job site located at 3766 Jeffery Ct. in Arnold MO. The employees were conducting roofing operations that exposed them to the hazard of falling from a heights greater than ten (10) feet.
