29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Commercial Construction located at 4445 Hugh Howell Road, Tucker, Georgia 30084, Scaffold: On or about June 30, 2025, the employer exposed employees to fall hazards of 13-feet, 2-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Commercial Construction located at 4445 Hugh Howell Road, Tucker, Georgia 30084, Roof:  On or about June 30, 2025, the employer exposed employees to fall hazards of 11-feet, 6-inches to the lower level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a) Commercial Construction located at 4445 Hugh Howell Road, Tucker, Georgia 30084, Ladder to roof:  On or about June 30, 2025, the employer exposed employees to fall hazards of over 11-feet, 6-inches to the lower level, in that the ladder side rails did not extend at least 3-feet past the roof landing to allow for safe access on and off the roof.
