Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On or about June 25, 2025, Sure Trac Construction Services LLC did not ensure employees were protected from fall hazards while performing roofing work at heights ranging from 20 to 30 feet above lower levels at a worksite located at 1708 Coupru Court in Saint Peters, Missouri.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On or about June 25, 2025, Sure Trac Construction Services LLC did not provide a training program for employees to recognize and minimize hazards associated with falls when working at heights of up to 30 feet above the lower level at a worksite located at 1708 Coupru Court in Saint Peters, Missouri.

29 CFR 1926.59: The employer had not developed a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) as incorporated by reference:  29 CFR 1910.1200(e)(1): Employers had not developed, implemented, and maintained at each workplace, a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets (SDSs), and employee information and training would be met.  On or about June 25, 2025, Sure Trac Construction Services LLC, did not develop, implement, and maintain at each workplace a written hazard communication program which at least described how the criteria specified in paragraphs (f) for labels and other forms of warning, (g) for SDSs, and (h) for employee information and training would be met. Gasoline a Category 1 flammable liquid and Tribuilt polyurethane sealant were being utilized by employees at a worksite located at 1708 Coupru Court in Saint Peters, Missouri. The employer did not have a written hazard communication program nor did they have SDSs available to employees.
