29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident:   a) At the worksite 32 Trafalgar Drive Shirley NY 11967. On or about June 24th, 2025 the employer failed to report the death of an employee within 8-hours of the incident. The fatality was reported to OSHA 06/26/2025.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
