29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   On or about July 1st, 2025, employees engaged in roofing operations located at 5503 North 207th Street, Elkhorn, Nebraska 68022, were exposed to fall hazards in that employees were observed working at heights greater than 10 feet 2 inches without protection against falling.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 1st, 2025, employees engages in roofing operations located at 5503 North 207th Street, Elkhorn, Nebraska 68022, were exposed to fall hazards in that employees were using an extension ladder for access and egress to and from the roof without having extended the ladder at least 3 feet above the upper landing surface .

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about July 1st, 2025, employees engaged in roofing operations located at 5503 North 207th Street, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer failed to provide training on how to use and inspect ladders to his employees prior to the employees using ladders on the jobsite.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about July 1st, 2025, employees engaged in roofing operations located at 5503 North 207th Street, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer failed to maintain certification records that document the employees working at elevation were trained on the hazards associated with performing residential roofing operations before allowing the employees to perform the work.
