OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards:  a) 1119 Monocacy St. Bethlehem Pa 18018,3rd floor, On or about July 1, 2025, the employer utilized two Sumner Roust-A-Bout, manually operated lifting devices, Model Numbers R-100 R-150, which were modified in that an unknown I-beam with unknown load capacity spanned 22ft between them. Attached to the I-beam, was an unknown Manual Trolley with unknown load capacity.  This was utilized to lift and place 350 pound plates into place on a stairwell.  Deficiencies include:  1. A missing trolley end stop on the load bearing I-beam;  2. Unknown I-beam load capacity; 3. Unknown trolleys' rated load capacity.  This exposed employees to struck by and crushing hazards.   Abatement Note: Among other methods, reasonable and feasible means of abatement for this hazard include:  1. Install trolley stop on each end of the I-Beams following the American Society of Mechanical Engineers (ASME) standard for Cranes and Monorails (with underhung trolley or bridge), ASME B30.17-2020, Section 17-1.9.2, Trolley Stops.  2. Only hang loads from beams with a known rated load capacity not exceeding the load.  3. Install and operate the trolley system per the American Society of Mechanical Engineers (ASME) standard for Overhead, Underhung and Stationary Hoists. ASME B30.16- 2017, Section 16-1.3.2, Support.   The Sumner Roust-A-Bout R Series Operator's Manual: (which the employer provided) Test load balance before lifting. Do not exceed rated load capacity.  Never use two Roust-A-Bouts to lift a load which exceeds the capacity of a single lift.  Do not use accessories with the Roust-A-Bout that are not supplied by the manufacturer.
