29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  On or about July 2, 2025 and times prior there to, at a new residential construction site, the employer did not ensure employees conducting roofing activities above 6 feet had a fall protection system in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  On or about July 2, 2025, and at times prior to, on the Southside of property the employer did not ensure ladder used at jobsite were extended 3 feet above landing.
