29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a) New residential construction located at 3646 South Bogan Road, Buford, GA 30519: On or about July 2, 2025, employees engaged in debris removal at ground level were exposed to struck-by hazards due to ongoing roofing activities directly overhead. Workers above were handling shingles and tools, creating a risk of falling objects to those below without adequate overhead protection.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  a) New residential construction located at 3646 South Bogan Road, Buford, GA 30519: On or about July 2, 2025, the employer exposed employees to struck-by hazards while operating a DeWalt circular saw and other equipment, in that protective eyewear was not provided while operating these tools while engaged in roof sheathing replacement activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Residential construction located at 3646 South Bogan Road, Buford, GA 30519:  On or about July 2, 2025, the employer exposed employees to fall hazards of 16-feet  to the lower ground level, in that a fall protection system was not provided while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Residential construction located at 3646 South Bogan Road, Buford, GA 30519:  On or about July 2, 2025, the employer exposed employees to fall hazards of 10 feet, in that the ladder used to access the roof of the structure was not extended at least three feet above the upper landing surface or secured to prevent accidental movement during use.
