29 CFR  1926.102(a)(1):Employees did not use eye or face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  The employer does not require the use of any form of eye protection during the installation of roofing material with pneumatic/air powered nail guns. This violation was most recently observed on or about July 3, 2025 at 1868 Nantucket Lane, Stow Ohio 44224, where an employee was not protected from struck by flying particle hazards while installing roofing material with pneumatic/air powered nail guns. To abate this violation, the employer must ensure that its employees installing roofing material are protected from struck by flying particle hazards, and that no employee is allowed to install roofing material while utilizing pneumatic/air power nail guns without adequate eye protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  The employer does not require the use of any form of fall protection during residential roofing work. This violation was most recently observed on or about  July 3, 2025 at 1868 Nantucket Lane, Stow Ohio 44225, where four employees were not protected from fall hazards of the working surface of the two-story roof that was approximately  9 feet and 21.3 feet to the ground. To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof 6 feet or more above lower levels without adequate fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer does not require the side rails of portable ladders used to gain access to an upper landing surface (roof) to extend at least 3 feet above the upper landing surface (roof).  This violation was most recently observed on or about July 3, 2025, at a jobsite located at 1868 Nantucket Lane, Stow Ohio 44221; where employees used a portable extension ladder to gain access to a roof during the installation of roofing material.  To abate this violation, the employer must ensure that its employees using portable ladders to gain access to upper landing surfaces such as roofs, extended the side rails of the ladder at least 3 feet above the upper landing surface or if it is not possible because of the ladder's length, secure the top of the ladder and provide a grasping device, such as a grabrail to assist employees mounting and dismounting the ladder, and that no employee is allowed to use portable ladders without being extended at least 3 feet above the upper landing surface or secured at the top and a grasping device provided.
