29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  a) On July 8, 2025, employees were provided with electrical tools and equipment connected by cord and plug without the use of an assured equipment grounding conductor program or ground fault circuit interrupters (GFCI's).  No abatement certification or documentation is required for this item.

29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access:  a) On July 8, 2025, employees were exposed to a fall hazard of approximately 18.1 feet while accessing and egressing  from a JCB S1930E electric scissor lift.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

No abatement certification or documentation is required for this item.
