29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) On or about, July 8, 2025, at the job site located at Bayview Road / Hwy 83, Mukwonago, WI 53149, the employer did not ensure that employees working on top a commercial wood structure while installing plywood roof decking were protected from an approximate 15.5 foot fall hazard to the lower level.

(a) On or about July  8, 2025, the employer did not ensure fall protection training was recorded with a written certification.
