29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  a) Worksite located at U.S. Interstate 287, mile marker 31.4, Harding Township, NJ and corporate location 15 Pine St, Kenvil, NJ  The employer did not report the death of a tow truck operator who was struck and killed during vehicle towing operations on July 2, 2025 approximately 11:45am.  The employer became aware of the death on July 2, 2025.  Violation occurred on or about July 2, 2025
