29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite: 415 Breckenridge St, Phoenixville, PA- On or about July 9, 2025, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system.  Employees were performing roofing approximately 20 feet above ground level without the use of a fall protection system, thereby exposing employees to fall hazards.  Shalom Roofing & Siding, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1776368, citation number 1, item number 2 and was affirmed as a final order on May 5, 2025, with respect to a workplace located at 649 Chestnut St, Pottstown, PA 19464.  Shalom Roofing & Siding, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1778142, citation number 1, item number 1 and was affirmed as a final order on March 11, 2025, with respect to a workplace located at 450 Prospect Street, South Orange, NJ 07079

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a) Jobsite: 415 Breckenridge St, Phoenixville, PA- On or about July 9, 2025, the employer did not ensure the side rails of a portable ladder used for access to an upper landing surface extended at least 3 feet above the upper landing surface being accessed. Employees utilized a portable ladder to access a second story roof without having side rails extended at least 3 feet above the landing surface, thereby exposing employees to fall hazards.  Shalom Roofing & Siding, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.1053(b)(1), which was contained in OSHA inspection number 1776368, citation number 1, item number 3 and was affirmed as a final order on May 5, 2025, with respect to a workplace located at 649 Chestnut St, Pottstown, PA 19464.
