29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jobsite, State Route 993 near Quality Way in North Huntington Township, PA :  On or about July 8, 2025, the employer failed to provide instructions to employees measuring the surface level of a bridge regarding the recognition and avoidance of unsafe conditions while a vacuum truck was in operation during a hydro-blasting process.
