29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention program as necessary: a) Jobsite, State Route 993 near Quality Way in North Huntington Township, PA : On or about July 8, 2025, the employer failed to initiate and maintain safety and health programs related to the work being performed at the jobsite. This included work rules, hazard detection and site safety enforcement to mitigate hazards associated with the removal of hydro demolition debris.
