29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) New construction located at 5415 Mira Vista Way Hoschton, GA 30528, Roof: On or about 7/9/2025, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that eye protection was not provided to employees operating these tools at 120 PSI while engaged in framing activities.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New construction located at 5415 Mira Vista Way Hoschton, GA 30528, roof: On or about 7/9/2025, the employer exposed employees to fall hazards of 10-feet, 2-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) New construction located at 5415 Mira Vista Way Hoschton, GA 30528, Roof: On or about 7/9/2025, the employer exposed employees to fall hazards of 10-feet, 2-inches to the lower ground level, in that the ladder was not extended three feet above the walking working surface to allow employees engaged in framing activities safe access/egress to the roof.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   a) New construction located at 5415 Mira Vista Way Hoschton, GA 30528, Roof: On or about 7/9/2025, the employer exposed employees to an approximate 8-foot fall hazard to the ground level below, in that a Werner step ladder was used improperly in a closed position to access and exit a job-made work platform.
