29 CFR  1926.50(c): At this job site, the employer did not have a person who was certified in first aid training (which includes Basic First Aid and CPR) available to render first aid.   On 06 July 2025, (3) three employees were exposed to an electrical hazard while repositioning two portable aluminum extension ladders, which were lashed together approximately 38 feet in height, at 40016 Highway P in Vandalia, Missouri. The employer did not ensure that a person who was certified in first aid training who was also available to render first aid was made available to employees. Two employees received an electric shock and one employee was electrocuted when the two lashed together portable aluminum ladders the employees were repositioning contacted an energized overhead high voltage powerline.

29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take:  On 06 July 2025, (3) three employees were exposed to an electrical hazard while repositioning two portable aluminum extension ladders lashed together approximately 38 feet in height, at 40016 Highway P in Vandalia, Missouri. The employer did not determine if employees could contact energized overhead powerlines, did not post warning signs, and did not advise the employees of the location of the energized overhead powerlines. The employees were not made aware of a 7,200 volt three phase overhead power distribution line.

The ladders were not designed to be fastened together to provide longer sections. Two employees received an electric shock and one employee was electrocuted when the two attached portable aluminum ladders the employees were repositioning contacted an energized overhead high voltage powerline.

29 CFR 1926.1060(a): The employer did not provide an adequate training program for each employee using ladders, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders:  On 06 July 2025, (3) three employees were exposed to an electrical hazard while repositioning two portable aluminum extension ladders lashed together approximately 38 feet in height, at 40016 Highway P in Vandalia, Missouri. The employer did not provide adequate ladder training to the employees related to appropriate ladder usage procedures to be followed and associated electrical hazards at the jobsite. Two employees received an electric shock and one employee was electrocuted when the two attached portable aluminum ladders the employees were repositioning contacted an energized overhead high voltage powerline.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  On 06 July 2025, a fatality occurred while employees were repositioning lashed ladders which made contact with an overhead powerline, at 40016 Highway P in Vandalia, Missouri. The fatality was reported to OSHA on 08 July 2025, by the Audrain County Sheriffs office and OSHA was not notified prior to 08 July 2025.
