Orlando, FL: employees were exposed to a 13 foot fall hazard while conducting roofing activities on a 4:12 pitched roof, in that, fall protection was not used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about July 10, 2025, at 2419 Skan Ct. Orlando, FL: employees were exposed to a 13 foot fall hazard while conducting roofing activities on a 4:12 pitched roof, in that, employees used an extension ladder to gain access to the roof and the siderails extend approximately 18 inches above the roof landing.

On or about July 10, 2025, at 2419 Skan Ct. Orlando, FL:  the employer did not prepare and maintain a written fall protection training program certification for employees exposed to fall hazards of 13 feet while working on a 4:12 pitch roof.
