29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about July 11, 2025, during the installation of roofing material at the job site located at 618 Crestview Ave., Akron, OH 44240; The employer did not ensure that employees were provided with or utilized safety eye glasses while using pneumatic nail guns for installing of roof shingles.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about July 11, 2025, during a residential construction roofing project located at 618 Crestview Ave., Akron, OH 44240: employees were exposed to a fall hazard of approximately 19 - 20 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure the extension ladder extended at least 3 feet above the upper landing surface.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about July 11, 2025, the employer did not ensure employees engaged in installing roofing material on a residential roof located a 618 Crestview Ave., Akron, OH 44240, approx. 19 - 20 feet above the ground were protected by guardrail systems, safety net systems, or personal fall arrest systems.  JR Roofing Services, was previously cited for a violation of this occupational safety and health 29 CFR 1926.501(b)(13), which was contained in OSHA inspection 1548820, Citation 2, Item 1 and was affirmed as a final order on September 13, 2021, with respect to a workplace located at 3790 North Santom Rd., Stow, OH, 44224.
