On or about July 11, 2025, during the construction of a corporate headquarters office tower complex located at 1350 West 3rd street, Cleveland, Ohio, the employer did not ensure the operator of a Clark powered Industrial truck (forklift) wore their seat belt personal restraint device (personal protective equipment) while operating the powered industrial truck to remove trash and debris from the worksite, exposing the operator to hazardous conditions that could result from being thrown and/or falling from the powered industrial truck.
