Southwest Area of Lot: On or about July 14, 2025, employees unloading vegetation debris were exposed to struck-by and caught-in hazards while working near and around a "Sany" model SY 365C LC excavator.

29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident.  a. On or about July 14, 2025, the employer did not report within 8-hours a work-related incident to OSHA resulting in the death of an employee which occurred on July 14, 2025. The fatality occurred at 8:10 a.m. and the Cobb County Medical Examiner reported the fatality to OSHA at 5:14 p.m. the same day.
