Location: Job site;  On or about 07/16/2025, employees were allowed to utilize a Genie SX-180 aerial lift which was set up on unstable and uneven cribbing with the back right tire hanging off the side of the cribbing it was positioned on.

29 CFR 1926.453(b)(2)(v): A safety harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.  Location: Job site;  On or about 07/16/2025, an employee working from an aerial lift was not protected from ejection hazards where personal fall restraint equipment was not worn or connected to the basket of the lift from a height of approximately 100 to 120-feet above the ground below.
