On or about July 14, 2025, at 2150 Dairy Rd. West Melbourne, FL: the employer did not ensure that cave in protection was installed in a 5 foot deep excavation in type C soil, while employees installed a concrete drain box and drain pipes, exposing employees to cave in and suffocation hazards.
