29 CFR  1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:  On or about July 17, 2025, during the installation of roofing material at a job site located at 625 Erie ST S., Massillon Ohio 44646; the employer did not ensure employees working on a roof of a commercial location were protected from a fall of approximately 22.2 feet by utilizing a fall protection system that was complete and set up adequately.

29 CFR  1926.502(f)(1)(iv): When the path to the point of access was not in use, a rope, wire, chain, or other barricade, equivalent in strength and height to the warning line, was not placed across the path at the point where the path intersected the warning line erected around the work area, or the was not offset such that a person cannot walk directly into the work area.   On or about July 17, 2025, employees were exposed to a fall of approximately 22.2 feet while performing roofing activities on a commercial roof located at 625 Erie ST S., Massillon Ohio 44646.  The employees had direct access to a portable ladder (point of access/egress) that was not adequately marked and or cordoned off via wire, rope, chain and or flagging system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about July 17, 2025, employees were exposed to a fall of approximately 22.2 feet while performing roofing activities on a commercial roof located at 625 Erie ST S., Massillon Ohio 44646.  The employees were using a portable ladder to access/egress the working surface of the roof that did not extended three feet above the landing area.
