29 CFR 1926.102(a)(1): The employer had not ensured each affected employee had used appropriate eye and face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  On or about July 16, 2025, Dr Roof LLC did not ensure each employee exposed to eye hazards from flying particles or debris, while operating pneumatic nail guns, utilized eye protection at a worksite located at 12280 County Road 3450 in Saint James, Missouri.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On or about July 16, 2025, Dr Roof LLC did not ensure employees were protected from fall hazards while performing roofing work at heights of 12-20 feet above the lower levels at a worksite located at 12280 County Road 3450 in Saint James, Missouri.
