29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or the basket when working from an aerial lift:  Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  a)  On July 16, 2025, employees were exposed to the hazard of falling approximately 25 feet to the ground while working from the JLG 740 AJ aerial lift without wearing personal fall arrest equipment or a full body harness with a lanyard attached to the boom or basket.    No abatement certification or documentation is required for this item.

29 CFR  1926.502(d)(6)(i):Snap hooks were connected  directly to webbing, rope or wire rope:  a) On or about July 16, 2025, employees performing commercial construction activities were not protected from a fall exposing employees to a fall of more than 25 feet.  Employees attached snap hooks directly to rope life lines, exposing employees to disengagement of snap hook, which could lead to a fall hazard of more than 25 feet.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.503(b)(1):The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  On or about July 16, 2025, the employer failed to document fall protection training provided to employees.  The employer did not certify fall training for employees performing commercial construction activities at heights exceeding six feet.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
