Winsupply Worcester MA Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1613939, citation number 1, item 1a, and was affirmed as a final order on March 8, 2023, with respect to a workplace located at 641 Middle St. Middletown, CT 06457  Winsupply Worcester MA Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1580208, citation number 1, item 1a, and was affirmed as a final order on September 7, 2022, with respect to a workplace located at 115 E. 9 Mile Rd. 29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition.  100 Southbridge St., Auburn, MA: On or about July 18, 2025, and at times prior, employees operated a Toyota forklift, model 42-6FGU15, that had two vertical support bars of the load backrest broken and missing and the forklift was not removed from service until restored to safe operating condition.
