29 CFR  1926.602(d): The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):   a) On or about July 18, 2025, at the educational institution located at 600 SE 8th St, Fort Lauderdale, FL, 33316, the employer did not provide training to employees who were exposed to struck and caught by hazards while operating powered industrial trucks (i.e. JLG -Model: 1055/ Serial No. 0160072624).

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   a) On or about July 18, 2025, at the educational institution located at 600 SE 8th St, Fort Lauderdale, FL, 33316, employees were exposed to an approximate 20-foot fall hazard while performing roof wood decking work without being protected from falling with a conventional fall protection system.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  a) On or about July 18, 2025, at the educational institution located at 600 SE 8th St, Fort Lauderdale, FL, 33316, employees were exposed to an approximate 20-foot fall hazard while performing roof wood decking work using a ladder that was not secured at its top to a ridgid support.
