29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  At 80 Elm Tree, Moscow Mills, MO 63362, on July 17, 2025 and at times prior thereto, employees were exposed to struck by and engulfment hazards from a not having a stairway, ladder, ramp or other safe means of egress located in a trench excavation that was 4 feet or more in depth so that it did not require more than 25 feet of lateral travel for employees.

At 80 Elm Tree, Moscow Mills, MO 63362, on July 17, 2025 and at times prior thereto, employees were exposed to struck by and engulfment hazards from having employees in an excavation that was over five feet and not protected from cave-ins by an adequate protective system.
