29 CFR  1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.    a) On or about 7/15/2025 at the site located at 839 NY-13 #7, Cortland, NY 13045: Employees used a Delta bench grinder, MFG # 1951, with a work rest of 3/8 inch exposing an employee to caught in and struck by hazards.
