Location: Job site;  On or about 07/24/2025, the foreman and employees were allowed to utilize pneumatic nail guns without proper eye/face protection to protect against potential flying objects.

29 CFR  1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  Location: Job site;  On or about 07/24/2025, the foreman and other employees were allowed to be exposed to electric shock and/or electrocution hazards while performing roofing activities in the direct vicinity of energized electrical service lines to the residence.

Location: Job site;  On or about 07/24/2025, employees were allowed to perform roofing work on a residential roof that was approximately 11-feet above the next lower level without any form of fall protection, such as guardrails, safety nets, or alternative protective measures.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder was not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter (4:1) of the working length of the ladder.  Location: Job site;  On or about 07/24/2025, employees and the foreman were allowed to utilize Werner portable extension ladders which were not erected in a manner to provide a 4:1 ratio of the working length of the ladder. These ladder were being utilized to gain access to the residential roof to conduct roofing activities.
