29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Location: Job site; On or about 07/14/2025, employees were allowed to be exposed to electric shock and/or electrocution hazards while utilizing an aluminum Werner portable extension ladder that was in the direct vicinity of energized electrical service lines, and was being used to gain access to the residential roof to perform roofing activities.

Location: Job site; On or about 07/14/2025, employees were allowed to perform roofing work on the roof of a residence between approximately 13- feet (chimney side) and 16-feet (garage side) above the ground below without any form of fall protection, such as guardrails, safety nets, or alternative protective measures.
