29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:  a) Jobsite located at or near S Northbank Place, Lot 19A-20H Philadelphia, PA - On or about July 30, 2025, the employer did not ensure all flexible cords that were used for temporary wiring were not passed through doorways or other pinch points, thereby exposing employees to electrical hazards.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a) Jobsite located at or near S Northbank Place, Lot 19A-20H Philadelphia, PA - On or about July 30, 2025, the employer did not ensure worn, or damaged flexible cord sets were removed from service.  Employees used flexible cord sets with external defects including but not limited to damaged wire insulation, thereby exposing employees to electrical hazards.

29 CFR 1926.453(b)(2)(v): 29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) Jobsite located at or near S Northbank Place, Lot 19A-20H Philadelphia, PA - On or about July 30, 2025,  the employer did not ensure employees working in an aerial lift wore a personal fall arrest or travel restraint system.  Employees installed siding in an aerial lift without the use of fall protection, thereby exposing employees to fall hazards.
