29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) ) On or about July 29, 2025, the employer did not ensure that the Haas Vertical Machining Center VF 5XT(model no. VF-5/40XT, serial no. 1098599) interlock on the on the operation door was working properly. The interlock had been bypassed which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. The employee had access to the rotating parts of the machine if the operator door would be opened and exposed the employee to struck-by and caught in-between hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On or about July 29, 2025, the employer had not provided employees working with hazardous chemicals such as flammable aerosols in the work area (including but not limited to OMNI-FILL Premium MasterBlend (FV16)) with effective information and training as outlined in 29 CFR 1910.1200(h)(2) through (h)(3). Information and training was not provided for the following elements: (b) On or about July 29, 2025, the employer had not provided employees working with hazardous chemicals such as CIMSTAR 60LF BLUE Metalworking Fluid with effective information and training as outlined in 29 CFR 1910.1200(h)(2) through (h)(3). Information and training was not provided for the following elements: (i) The requirements of 29 CFR 1910.1200, (iii) Methods and observations used to detect the presence or release of hazardous chemicals in the work area, (iv) The physical and health hazards associated with flammable aerosols, (vi) The measures employees can take to protect themselves from identified physical and health hazards associated with flammable aerosols such as safety procedures, emergency procedures, safe work practices, and PPE, and (vii) Explanation of container labeling systems.
