VF-5/40XT, serial no. 1098599) interlock on the on the operation door was working properly. The interlock had been bypassed which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. The employee had access to the rotating parts of the machine if the operator door would be opened and exposed the employee to struck-by and caught in-between hazards.
