29 CFR  1910.110(b)(6)(ii):29 CFR 1910.110(b)(6)(ii): Each individual container was not located the minimum specified distance from an important building or group of buildings in accordance to Table H-23:  Yard area by the White Wood Shed:  On or about July 25, 2025, employees were exposed to fire and explosion hazards while working in the yard area by the white wood shed.  There were multiple LP Gas Tanks with an aggregate amount of over 501 gallons, within 20 feet from the white wood shed.  Abatement certification is required of this item in accordance with 29 CFR 1910.1903(c).

There were multiple LP Gas Tanks.  Employees were burning off residual liquified petroleum gas, a flammable gas, using a lighter and a portable propane burner.  Abatement certification is required of this item in accordance with 29 CFR 1910.1903(c).

Employees were burning off residual liquified petroleum, a flammable gas, using an ignition source, such as a lighter, and a portable propane burner.  Liquefied petroleum gas containers in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering from unauthorized, persons. 29 CFR  1910.132(a):Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about July 25, 2025, employees working in the bulk yard storage area were burning off residual liquified petroleum gas, a flammable gas, using a lighter and a portable propane burner.  The employer failed to provide protective equipment, such as but not limited to, flame retardant clothing, to employees when exposed to fire hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard(s) identified. The employer failed to: 1. Select the appropriate PPE identified in the hazard assessment;  2. Communicate selection decisions to each affected employee;  3. Ensure that the selected PPE properly fits each affected employee.  On or about July 25, 2025, employees were exposed to fire and explosion hazards, while working in the bulk yard storage area were burning off residual liquified petroleum gas, a flammable gas, using an ignition source and a portable propane burner.   The employer did not select or use the type of personal protective equipment that would protect the affected employee from the hazards identified. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
