29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) New residential construction located at 240 Westlyn Way, Winder, GA 30680: On or about 07/29/2025, the employer exposed employees to fall hazards of 10 feet to the lower ground level, in that the ladder used to access the roof of the structure was not extended at least three feet above the upper landing surface or secured to prevent accidental movement during use.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located at 240 Westlyn Way, Winder, GA 30680: On or about 07/29/2025, the employer exposed an employee to struck-by hazard while operating pneumatic nail gun, in that protective eyewear was not provided to an employee engaged in roof sheathing and framing activities.  Gustavo Dimas Binzha was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1728503, citation number 1, item number 1 and was affirmed as a final order on 05-07-2024, with respect to a workplace located at 109 Silver Maple Lane, Lot #305, Warner Robins, GA 31093.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) New residential construction located at 240 Westlyn Way, Winder, GA 30680: On or about 07/29/2025, the employer exposed an employee to fall hazards of approximately 10-feet to the lower level, in that a fall protection system was not provided while engaged in framing and sheathing activities on a 6/12  pitched roof.  Gustavo Dimas Binzha was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1728503, citation number 1, item number 2-b and was affirmed as a final order on 05-07-2024, with respect to a workplace located at 109 Silver Maple Lane, Lot #305, Warner Robins, GA 31093.
