The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) Painter Hall- On or around July 28, 2025, at the job site, an employee was utilizing a body belt that lacked an attachment point centered on the wearer's back. Instead, the safety belt was secured at the front attachment point, located near the abdomen area.

Chavez Roofing,  LLC. was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA Inspection #1807956, Citation #1, Item # 1, issued on May 1, 2025, with respect to a workplace located at 11775 County Line Road Madison, AL 35758 which became a final order on May 28, 2025.
