29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 209 Westchester Avenue, Portchester - NY: Employees were exposed to fall hazards of approximately 20 and 30-feet while using extension ladders to access a roof. The extension ladders that were used, were only extended approximately 1-foot (20 foot ladder) and 1 & 1/2 foot (30 foot ladder) above the landings and not the required 3-feet, on or about, June 17th, 2025.
