29 CFR  1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  Mechanic shop:  On or about August 1, 2025, relocatable power taps (power strips) were series-connected (daisy-chained) to extension cords or other power strips not designed for its intended purpose was providing electrical power to various mechanic shop related equipment, lighting, air compressor, vehicle lift, 30 ton lift, and garage doors for employees to use, exposing them to electrical, stuck by, caught in between hazards.

Mechanic shop:  On or about August 1, 2025, flexible cords were used for the following;  A) Flexible cords (Extension cords) were used as a substitute for fixed or permanent wiring while energizing various mechanical shop related equipment, lighting, air compressor, vehicle lift, 30-ton lift, garage doors.  B) Flexible cords (Extension cords) were run through a wall near the ceiling by the door way entrance while energizing various mechanic shop related equipment and lighting.  C) Flexible cords (Extension cords) were run through a door way entrance to another room while energizing various mechanic shop related equipment and lighting.  D) Flexible cords (Extension cords) were attached with staples and tie wraps to building surfaces while energizing various mechanical shop related equipment, lighting, air compressor, vehicle lift, 30-ton lift, garage doors.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  Mechanic Shop/Doorway:  On or about August 1, 2025, three junction boxes that were interconnected near the doorway, had exposed insulated wires from a cable without the proper devices and fittings for strain relief to prevent pulling while energizing various mechanic shop related equipment, lighting, air compressor, vehicle lift, 30-ton lift, and garage doors that were being used by the employees to perform mechanical work exposing them to electrical hazards.
